Why your gums bleed when you brush

Almost every week, someone sits in our chair and says the same thing: “the gums bleed a little when I brush, but that is normal, right?”

It is common. It is not normal.

Bleeding means inflammation

Healthy gums do not bleed when you brush them, in the same way healthy skin does not bleed when you wash it. When gums bleed, there is inflammation — and the cause is almost always a layer of plaque sitting where the gum meets the tooth.

Plaque is soft for about a day or two. After that it hardens into tartar, and once it hardens, no toothbrush will remove it. That hard deposit keeps the gum irritated permanently, so the bleeding continues no matter how carefully you brush.

This early stage is called gingivitis, and it is completely reversible.

The mistake that makes it worse

The instinct is to avoid the bleeding spot. People brush around it, brush more gently there, or skip it altogether.

That is exactly backwards. Less cleaning at that spot means more plaque, more inflammation, and more bleeding. The area that bleeds is the area that needs cleaning most.

The one genuine exception: if you are scrubbing hard with a medium or hard brush, you can damage the gum mechanically. Use a soft brush, hold it at around 45 degrees to the gumline, and use small circles rather than a sawing motion. Pressure is not what cleans — contact time is.

When gingivitis becomes something worse

If inflammation continues for months or years, it stops being confined to the gum. It reaches the bone that holds the tooth in its socket. That stage is periodontitis, and it behaves differently:

• Gums start pulling away from the teeth, so teeth look longer
• Small pockets form between gum and tooth, trapping more bacteria
• Persistent bad breath that mouthwash does not fix
• Teeth feel slightly mobile, or start to drift apart
• Eventually, teeth loosen enough to be lost

Here is the important part: bone lost at this stage does not grow back on its own. Treatment can stop the loss and stabilise things, but it cannot rebuild what has gone. This is why the bleeding stage matters so much — that is the window where everything is still reversible.

What to do this week

1. Switch to a soft-bristled brush if you are not on one already.
2. Brush twice, properly. Two minutes, angled at the gumline. Most people brush for under 45 seconds.
3. Clean between the teeth with floss or interdental brushes. A toothbrush physically cannot reach the surfaces between teeth, and that is where gum disease usually starts.
4. Get a scaling done. Once tartar has hardened, this is the only way it comes off. It takes one appointment.

If the bleeding stops within two weeks of doing all four, it was early-stage gingivitis and you have caught it. If it does not, get it examined — that usually means the deposits have gone below the gumline, where a cleaning appointment can reach and a toothbrush cannot.

The health connection people do not expect

Gum disease and diabetes make each other worse in both directions. Uncontrolled blood sugar weakens the gum’s ability to fight infection, and chronic gum infection makes blood sugar harder to control.

If you are diabetic and your gums bleed, treat it as two problems that share one solution. It is one of the practical advantages of being treated inside a multispeciality clinic — that conversation can involve both departments without you travelling anywhere.
